Chronic low back pain like that described by the veteran in this case report is quite common and can be quite disabling. Spinal manipulation is endorsed for the management of chronic low back in quite a few guidelines supported by such groups as the American Pain Society and the American College of Pain Physicians. (2,3) With chronic low back pain, central sensitization – the effect of repetitive nerve stimulation has on the central nervous system - is purported to play a role in chronicity of pain.  Spinal manipulation’s benefit may well come from its effect on this sensitization. SPINAL MANIPULATION BETTER FOR CHRONIC LOW BACK PAIN…REGARDLESS OF AGE

A systematic review and meta-analysis of randomized controlled trials (47 RCTs and 9211 participants) of spinal manipulation treatment for chronic low back pain reported that spinal manipulation produced similar effects to other recommended therapies of chronic low back pain (like exercise and drug therapy), was superior to non-recommended intervention for short-term improvement, and had limited, mild-to-moderate, passing adverse effects. (5) Another such meta-analysis determined that spinal manipulation appeared to be a good alternative for pain relief and function improvement for chronic low back pain sufferers. (6) In a meta-analysis specifically focused on older (ages 65-91 y/o) chronic low back pain sufferers, the same deductions were found: spinal manipulation should be thought to be a positive treatment for the older chronic low back pain sufferer for pain relief and function improvement. (7) Most spinal manipulation is delivered by chiropractors according to the National Center for Complementary and Integrative Health.
