Leg Length Discrepancy and Back Pain: How Chiropractic Care Can Help

September 29, 2026

Do you notice one pant leg dragging more than the other, or a sore back after hours on your feet? You could be living with a leg length discrepancy (LLD), a common difference in the length of your legs. Slight differences are normal, but when one leg is measurably shorter, the pelvis can drop toward that side and the spine bends to adjust. Over time, that lopsided base can stress the joints, discs, and muscles of the lower back.

WHAT THE RESEARCH SAYS ABOUT LEG LENGTH DISCREPANCY

Studies show that measuring a leg length difference requires care and consistency. Tipton, Sutherland, and Schwarzkopf (1) reported that measurements from a standard pelvic X-ray differed significantly from actual leg length, and advised looking for any preexisting discrepancy before hip replacement surgery. Kadhim, Kelly, and Sabharwal (2) found that three full-length standing X-ray methods yielded comparable results, but suggested using the same technique every time, particularly when the legs bow or angle. The source of the difference matters as well. Tolk, Eastwood, and Hashemi-Nejad (3) linked leg length differences in Perthes' disease to the top portion of the thigh bone near the hip. Birch, Makarov, and Jo (4) showed that when one part of a child's leg is shortened, the unaffected part of that same leg typically doesn't grow longer to make up the difference. You can trust your chiropractor to figure it all out for you!

HOW CHIROPRACTORS ADDRESS LEG LENGTH AND BACK PAIN

This is where your chiropractor plays a key role. Chiropractic's greatest contribution is noticing the discrepancy and understanding how it changes spinal mechanics in ways that may cause pain. Your assessment might include looking at pelvic level, posture, and gait, along with X-rays if necessary. Your care plan may then include gentle spinal adjustments, flexion-distraction to reduce pressure on irritated joints and discs, individualized exercises, and heel lifts when appropriate. If a discrepancy requires orthopedic care, your chiropractor can certainly help coordinate that referral.
